Postoperative Bleb Management with Topical Mitomycin-C

چکیده
Despite various developments in many fields of ophthalmology, the surgical therapy of open-angle glaucoma has remained mostly unchanged over the last 30 years. Ever since the introduction of intraoperative mitomycin and postoperative subconjunctival injections of 5-fluorouracil (5-FU), trabeculectomy has not markedly been altered. These facts stand in contrast to the need for a more efficient surgical therapy for open-angle glaucoma, since one of the main disadvantages of filtering surgery is the lack of enduring success. Largescale retrospective studies have suggested that filtering blebs become non-functional in at least 50% of patients, 5 years after surgery. This clearly demonstrates the necessity for developing techniques to reduce postoperative scarring in filtering blebs. I n t h e c u r r e n t i s s u e o f J o u r n a l o f Ophthalmic and Vision Research, Pakravan and colleagues1 have examined the effect of postoperative topical mitomycin 0.02% eye drops on the success of trabeculectomy surgery and compared the results with that of repeated postoperative subconjunctival 5-FU injections. In their randomized study, for which the authors have to be commended, the authors found that topical mitomycin compared favorably with subconjunctival 5-FU injections. Since eye drops are easier to apply and cause less pain, topical application of mitomycin appears to be a valid alternative to subconjunctival 5-FU injections in the postoperative period. Certain questions however remain. First, interestingly, the toxicity of mitomycin applied topically on intact conjunctival epithelium is considerably less than that applied on bare sclera. Future studies may address whether this disparity is due to different durations of application (drops have relatively short period of contact with the ocular surface), variations in effective concentrations of the agent (drops may be diluted by tears), and/or due to the protective effect of an intact conjunctival epithelium. Second, it is not clear yet whether prolonged topical application of mitomycin may cause corneal stem cell insufficiency leading to problems in corneal epithelialization or dry eye due to damage to conjunctival goblet cells. Third, the risk of systemic side effects of mitomycin eye drops due to absorption through the nasolacrimal duct mucosa may additionally be addressed. In conclusion, the study by Pakravan et al may show new possibilities and open up a new avenue in the postoperative care of patients after routine trabeculectomy. Since the application of mitomycin eye drops is considerably more convenient and probably less costly, it may indeed be an alternative to postoperative subconjunctival injections of 5-FU.

Needling augmented with topical application of mitomycin C for management of bleb failure.
PURPOSE To study the effect of topical application of mitomycin C on enhancing the efficacy of needling in the management of bleb failure. PATIENTS AND METHODS Thirty-six eyes of 32 consecutive patients with an intraocular pressure over 21 mmHg, without bleb or with a thick, flat bleb after the second postoperative month after trabeculectomy, were included in the study. Needling with mitomyci...

PURPOSE To investigate the safety and synergistic effect of topical bevacizumab after trabeculectomy surgery with mitomycin C (MMC). METHODS In this prospective, non-randomized, comparative interventional study, 40 eyes from 40 patients with uncontrolled open-angle glaucoma were studied after they underwent primary trabeculectomy with mitomycin C (0.02% for 2 min). Following the procedure top...
متن کاملISRCTN12125882 - Influence of topical anti-VEGF (Ranibizumab) on the outcome of filtration surgery for glaucoma - Study Protocol
BACKGROUND Excessive wound healing, with scarring of the episcleral tissue or encapsulation of the filtering bleb is the main reason for failure in trabeculectomy. Ranibizumab, an inhibitor of the Vascular Endothelial Growth Factor (VEGF), is seen as a promising candidate to prevent or treat extensive wound healing. We describe the design of a two phased study, i) assessing the local tolerabili...

This case report illustrates the use of ExPRESS mini-shunt in a pediatric glaucoma patient. We describe the management of steroid-induced glaucoma with ExPRESS mini-shunt in a 9-year-old boy with allergic keratoconjunctivitis. The intraocular pressure of both of his eyes was uncontrolled with medical and laser treatment. Both eyes were treated with ExPRESS mini-shunt and mitomycin-C. Transient ...
متن کاملPrevention of Ocular Scarring Post Glaucoma Filtration Surgery Using the Inflammatory Cell and Platelet Binding Modulator Saratin in a Rabbit Model
CLINICAL RELEVANCE Late complications can occur with use of current antimetabolites to prevent scarring following glaucoma filtration surgery (GFS). Safer, more targeted, anti-fibrosis agents are sought. OBJECTIVES The protein saratin has been shown to exhibit anti-fibrotic and anti-thrombotic properties in response to injury, but had not been used for glaucoma surgery. The goal of this study...
